Job summary

CVS Health is seeking a Behavioral Health Patient Engagement Specialist for a remote, patient-facing role. You will coordinate patient intake, schedule appointments, verify eligibility, manage referrals, and match patients with providers, acting as a liaison between patients and care teams.

Qualifications

• 2+ years in healthcare coordination, patient access, intake coordination or related roles.
• Strong verbal and written communication skills to engage patients professionally.
• Ability to manage multiple worklists, priorities, and follow-up tasks independently.
• Proficient with multiple channels of communication: phone, email, chat, etc.
• Attention to detail and accurate documentation of outreach activity.
• Comfort in a metrics-driven environment.
• Ability to work remotely with a private workspace.
• Proficiency with productivity/reporting platforms.

Responsibilities

• Conduct preliminary screening and assessment of individuals seeking services within established timeframes.
• Manage targeted patient outreach to support intake conversion and engagement.
• Use strong communication to build trust with scheduled, prospective, inactive, or overdue patients to encourage appointments.
• Assist patients with understanding next steps, scheduling needs, and readiness.
• Proactively manage provider panels and waitlists to maximize access and productivity.
• Support provider panels with high-risk lists, overdue follow-ups, re-engagement, and outreach.
• Initiate targeted outreach campaigns: intake conversion, no-show reduction, re-engagement, provider outreach.
