Job summary

EverCare Mobile Health is seeking a Behavioral Health Case Manager / CBRS Specialist to deliver community-based services in residential and long-term care settings around Pocatello, Idaho.

You will provide CBRS/skills building, coordinate with treatment teams, complete documentation, and ensure services align with individualized goals while maintaining HIPAA privacy and professional boundaries. This is a full-time, hourly, non-exempt role with travel within communities.

Qualifications

• Bachelor's degree in a health or human services field from an accredited institution.
• Ability to meet credentialing and supervisory-protocol requirements and to participate in supervision.
• Valid driver's license, reliable transportation, and HIPAA/compliance knowledge.

Responsibilities

• Provide CBRS/skills building consistent with treatment goals and authorized services.
• Coordinate with treating clinicians and care teams to integrate CBRS with clinical treatment.
• Complete progress notes, service documentation, and related records on time.
