Job summary

Synapticure is seeking a Behavioral Health Care Manager to support mental health needs of individuals and families affected by neurodegenerative diseases. You will provide care coordination under clinical oversight, implement the CoCM program, and collaborate with senior care managers, medical providers, patients, and partners to improve mental health outcomes.

Responsibilities include screening, education, coordinating with providers, and maintaining detailed records in Synapticure’s EHR.

Qualifications

• Active independent clinical license (LCSW, LMFT, LPC, or equivalent) good for independent practice.
• 0-2 years of clinical case or care management experience welcomed for new licensure.
• Strong organizational, communication and interpersonal skills.
• Passion for serving vulnerable populations with patient-centered care.

Responsibilities

• Fulfill BHCM responsibilities within the CoCM program following UW AIMS Center guidelines.
• Coordinate care with medical providers and other mental health professionals.
• Screen and assess patients for mental health disorders; ensure follow-up care.
• Deliver education to patients and caregivers about mental health options.
• Monitor symptoms and outcomes; adapt care plans under supervision as needed.
• Provide evidence-based behavioral interventions (motivational interviewing, behavioral activation, problem-solving).
• Participate in case reviews with psychiatric consultants to support treatment plans.
• Facilitate referrals to external services as clinically indicated.
• Maintain accurate records in the EHR and communicate with the care team.
• Assist in relapse prevention planning and build trust with patients and families.
• Foster relationships with patients, caregivers, and partners to ensure continuity of care.
