Job summary

Ennoble Care is seeking an experienced Full-time Behavioral Health Integrated Care Manager (Hybrid) to service our patients within and around the Northern, VA area. This role will be primarily remote with occasional travel to patients' homes or facilities.

You will screen and assess patients, coordinate with medical providers, document progress in the EHR, and participate in weekly caseload reviews. The position requires MSW, behavioral health experience, and strong time management.

Qualifications

• MSW minimum required.
• Preferred licensure or provisionally licensed.
• Behavioral Health experience and knowledge.
• Must be a licensed driver with insured automobile.
• Ability to communicate effectively, both verbally and in writing.
• Time management skills.

Responsibilities

• Support the mental and physical health of patients on an assigned caseload.
• Screen and assess patients for common mental health and substance use disorders.
• Provide patient education about health and treatment options.
• Track treatment response and monitor patients for changes.
• Assist with psychotropic medication management by collaborating with specialists.
• Provide brief behavioral interventions using evidence-based techniques.
• Facilitate referrals to psychosocial treatments as clinically indicated.
• Participate in weekly caseload consultations with the psychiatric consultant.
• Document progress in EHR and other required systems.
