Job summary

Ennoble Care is seeking an experienced Full-time BHI Specialist (Hybrid) to service patients in and around the Northern, VA area (Arlington, Manassas, Alexandria). The role is primarily remote with occasional travel to patients' homes or facilities.

You will support mental and physical health care for a defined caseload, screen and educate on disorders, track outcomes, and coordinate care with providers. Proficiency in Excel/Word and EMR use is expected.

Qualifications

MSW required
Behavioral Health experience preferred
Licensed driver with insured vehicle
Excellent written and verbal communication
Ability to use Excel/Word daily

Responsibilities

Support mental and physical health care for an assigned patient caseload.
Screen and assess for mental health and substance use disorders; ensure engagement and follow-up.
Educate patients about disorders and treatment options.
Track treatment response and monitor clinical status via in-person or calls.
Coordinate medication management with providers and consultants.
Provide brief behavioral interventions (e.g., behavioral activation, problem-solving).
Facilitate referrals for external psychosocial services as needed.
Document progress in EHR and registry; share updates with care team.
Attend caseload consultations and relay recommendations to medical providers.
