Qualifikationen

• Requires Master's degree in Behavioral Health or Social Work or Nursing and 2–4 years related experience.
• Licensed Behavioral Health Professional or RN may be required for AZ Health Plan with BH experience.
• Must reside in Arizona.

Aufgaben

• Develops, assesses, and facilitates care management for mental/behavioral health needs.
• Evaluates member needs and develops care/service plans for best outcomes.
• Performs telephonic, digital, in-home visits to assess needs and coordinate resources.
• Identifies providers and community resources for care and ensures accessibility.
• Monitors plans and outcomes, adjusting care as needed and reporting changes.
• Facilitates coordination between members, families, and care teams.
• Documents member information to comply with state, federal, and payer regulations.
• Educates members and families on procedures, options, and benefits.
• Provides feedback to leadership to improve care delivery cost-effectively.
• Performs other duties as assigned and complies with policies.
