The Care Manager Clinician coordinates services, conducts holistic assessments, and collaborates with members, providers, and health plans to promote engagement and recovery. This role requires an unrestricted clinical license and 3–5 years of direct care experience in behavioral health or substance use

Qualifications

• Current, unrestricted state license to independently practice in the applicable state(s).
• 3-5 years of direct clinical care experience in behavioral health or substance use treatment.
• Ability to pass background check and related screenings.
• Strong clinical judgment and case management skills.
• Excellent verbal and written communication with empathy and clarity.

Responsibilities

• Coordinate behavioral health services to ensure high‑quality care and promote engagement and recovery.
• Conduct holistic assessments and develop individualized care plans.
• Coordinate care transitions and aftercare with members, providers, and health plan teams.
• Engage members and providers to improve engagement and continuity of care.
• Serve as the single point of contact for information exchange and discharge planning.
• Ensure timely documentation and authorization in line with medical necessity and policies.
• Participate in audits, quality improvement, and ongoing professional development.
