Job summary

UnitedHealth Group is seeking a Behavioral Health Advocate in Lucas County, OH. The role is home-based with about 35% field work visiting members in homes, hospitals, or community settings, and remote work is possible if located near Toledo, OH.

You will coordinate care, educate on conditions and medications, and help transition members back home while traveling locally. The position requires licensure and 2+ years of mental health experience.

Qualifications

• Master's degree in psychology, social work, counseling or related mental health discipline.
• Proficient computer skills including MS Word and Outlook.

Responsibilities

• Visit Medicaid members in their homes or other settings.
• Provide a complete continuum of quality care through close communication with members.
• Support members with condition education, medication reviews, and connections to resources.
• Assist members with the transition from a care facility back to their home.
