Job summary

CenterWell Home Health, part of Humana Inc., seeks an AVP, Clinical Product & Strategy to set the strategic direction for clinical programs that improve Stars, affordability, risk adjustment, and member outcomes across 400k patients in 37 states.

The leader will translate enterprise priorities into scalable home-based care models, build business cases, launch pilots, and scale programs in collaboration with Home, Pharmacy, Primary Care, and Humana teams.

Qualifications

• Bachelor’s degree in healthcare, business, or related field; Master’s preferred.
• At least 10 years of progressive healthcare leadership experience.
• Minimum 7 years leading managers and professional teams.

Responsibilities

• Own MA value creation strategy: develop roadmap for clinical programs improving Stars, affordability, risk adjustment, and outcomes.
• Oversee clinical program execution with day-to-day leadership and cross-functional coordination.
• Improve Quality and Stars performance by closing gaps with clinical, analytics, and MA teams.
• Drive appropriate Home Health utilization and demonstrate network savings and impact.
• Reduce avoidable costs by scaling interventions reducing hospitalizations and readmissions.
• Advance member engagement and longitudinal care beyond a single Home Health episode.
• Expand high-risk member services with new home-based care models and pilots.
• Improve ecosystem integration across CWHH, PCO, Rx, and Humana.
