Job summary

Ohio District 5 Agency On Aging is seeking a dedicated professional to perform assessments and screenings to link individuals with needed services. The role requires building connections with individuals, caregivers, and community agencies while navigating program specifications and timelines.

Responsibilities include conducting Level of Care and Comprehensive assessments, documenting thoroughly, and coordinating services across funding sources.

Qualifications

• Must be licensed to practice in Ohio (RN, LPN, or social worker).

Summary

This position is established for the purpose of performing assessment and screening to link individuals to needed services.

Essential Functions

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

1. Conducts phone calls and in-person contacts with individuals in accordance with program specifications and time frames.
2. Assists individuals in gaining access to any necessary services regardless of funding source.
3. Establishes initial service needs and ensures the prevention of service duplication.
4. Networks with informal caregivers to obtain necessary information and coordinate formal and informal services as appropriate.
5. Conducts Level of Care and Comprehensive assessments as needed.
6. Maintains all appropriate documentation in formats and time frames prescribed by the PAA, OHCW, ODA and the Ohio Department of Medicaid.
7. Maintains job functions as provided by and unique to this distinct work unit, separate from other units and reporting up a separate supervisory chain.
8. Understands the Agency’s Conflict of Interest Policy and complies with the Ohio Aging Network’s requirements for disclosure.
9. Participates in training to maintain and improve job skills.
10. Assists with and completes special projects and activities as assigned.
11. Performs duties with respectful communications & behaviors.

Requirements

1. Ability to work from home.
2. Ability to relate well with all individuals, community agencies, and Area Agency staff.
3. Strong verbal and written communication skills.
4. Knowledgeable in Microsoft Word and Excel.
5. Ability to communicate professionally, respectfully and effectively.
6. Ability to make sound decisions and demonstrate critical thinking skills.
7. Ability to prioritize, organize and maintain accuracy in a flexible job environment.
8. Successful completion of the Ohio Department of Aging certification required within 6 months of employment.

Eligibility Requirements

Registered Nurse, LPN or Licensed Social Worker holding an active license to practice in the State of Ohio.
