Responsibilities

• Provide telephonic and digital case management to health plan members.
• Coordinate care across primary, specialty, behavioral health, pharmacy, and community resources.
• Ensure timely follow-up after emergency department or hospital discharge.
• Promote medication adherence and chronic disease self‑management.
• Maintain privacy and adhere to regulatory standards.
