Job summary

Main Line Health, Inc. is seeking an RN Ambulatory Care Coordinator for a 100% remote role. You will coordinate patient care, monitor outcomes, and perform telephonic outreach to assess needs, recognize early symptoms, and promote self-management.

The ideal candidate holds an ASN and preferably a Bachelor’s degree, and must have an active RN license. Tuition reimbursement up to $6,000 annually is offered to support education and career growth.

Qualifications

• Experience coordinating care across providers, payers, and practice staff.
• Ability to conduct age-appropriate clinical assessments via telephonic outreach.
• Strong communication and patient education capabilities.

Responsibilities

• Coordinate care for patients and monitor outcomes and utilization of services.
• Conduct telephonic outreach focusing on assessments, early symptom recognition, and self-management.
