Sentara Norfolk General Hospital is seeking a dedicated Advanced Practice Provider to join the Case Management Pharmacy Program. This full-time role offers the flexibility to work from home or on-site in Norfolk, VA, with a schedule Monday–Friday, 8:30AM–5:00PM and no weekend shifts.

The APP will perform health risk assessments, re-write prescriptions, and manage about 150 patients monthly, primarily within Sentara Medicaid.

Qualifications

• 1 year experience required as APP in Family Medicine
• Experience in Rheumatology or Infectious Disease preferred
