Job summary
Sentara Norfolk General Hospital seeks a full-time Advanced Practice Provider (APP) to join the Case Management Pharmacy Program. This role supports health risk assessments, rewrites prescriptions, and reviews a high patient load from a hybrid setting that includes hospital and remote work.
Ideal candidates have 1 year of APP experience in Family Medicine; experience in Rheumatology or Infectious Disease is preferred.
Qualifications
- 1 year experience as APP in Family Medicine.
- Experience in Rheumatology or Infectious Disease preferred.
Responsibilities
- Duties include health risk assessments and rewriting prescriptions.
- Review 150 patients per month.
- Work from home or at Sentara Norfolk General Hospital.
