Qualifications

• Unrestricted NP/PA license with national board certification (AANP/ANCC/AACN for NP or NCCPA for PA).
• Active DEA license.
• Active national provider number (NPI) and Medicare provider number.
• Master of Science in Nursing as NP or equivalent.
• Experience in geriatric, hospice, or palliative care preferred.

Responsibilities

• Conduct acute, follow-up and transition of care encounters.
• Collaborate with primary providers for medical management, goals of care, care transitions, med reconciliation.
• Educate care teams and maintain relationships with patients, facilities and partner providers.
• Perform other duties to support high-quality patient-centered care.
