Job summary

Devoted Health, Inc. is seeking a Fully Remote Advanced Practice Provider to support the Gold Care program. The APP will manage the clinical trajectory of complex geriatric patients using telehealth and collaborate with an interdisciplinary team across care settings.

Requirements include an active RN and APRN or PA license in GA, PA, or OH, 3+ years as an APP in geriatrics or related fields, and proficiency with telehealth and EHR systems. BLS is required at hire and maintained during employment.

Qualifications

• Active RN and APRN or Physician Assistant license in GA, PA, or OH at time of hire.
• 3+ years as an APP with experience in primary care, geriatrics, or palliative care.
• Proficient with telehealth technology and electronic health records (EHR).
• Active BLS at time of hire and maintained during employment.

Responsibilities

• Deliver care to complex patients via video telehealth with an interdisciplinary team.
• Guide medical management and plan of care based on patient priorities and 5M framework.
• Coordinate with PCP, specialists, and Devoted teams, including pharmacy.
• Maintain accurate medical records and ensure privacy and compliance.
