Job summary

Devoted Health is seeking an Advanced Practice Provider for the Gold Care program, focused on complex geriatric patients. You will drive medical management, coordinate with PCPs and specialists, and conduct comprehensive assessments via telehealth to support high-need members across multiple geographies.

The role requires an active RN and APRN/PA license, 3+ years of APP experience in geriatrics or related fields, and fluency in telehealth/EHR tools.

Qualifications

Active RN and APRN or Physician Assistant license required with ability to obtain multiple state licenses.
Three or more years of APP experience in primary care, house calls, internal medicine, geriatrics or palliative care.
Proficiency with telehealth technology and electronic health records (EHR).
Active BLS at time of hire and maintained during employment.

Responsibilities

Deliver exceptional care to a cohort of complex patients across geographies via video telehealth and interdisciplinary teams.
Oversee medical management of patients using the 5M framework during Gold Care tenure.
Collaborate with PCPs, specialists, and other Devoted teams to coordinate care and create a shared care plan.
Perform comprehensive assessment visits, post-acute transition support, medication review, and quality-of-care interventions as needed.
Maintain accurate patient records and ensure compliance with privacy and telehealth regulations.
