Accompany Health, Inc. seeks a licensed NP or PA for a Virtual Float Advanced Practice Clinician role. You will deliver high-quality virtual care across patient panels, managing triage, care coordination, and follow-up with support from Health Advocates.

Shift patterns include 8am–5pm, Monday–Friday, in a flexible, remote setting. Applicants must hold an active NP or PA license, with 3+ years in adult primary care or related fields, and be proficient with telehealth platforms and EMR

Qualifications

• Active NP or PA license required.
• 3+ years in adult primary care, family medicine, urgent care, or emergency medicine preferred.
• Experience with telehealth platforms and virtual care workflows.
• Strong clinical assessment and decision-making skills.

Responsabilités

• Deliver virtual care visits for assigned patient panels.
• Perform triage, care coordination, and follow-up in a hybrid/virtual setting.
• Document care plans and orders accurately in the EMR.
• Coordinate with Health Advocates and other care teams to ensure continuity of care.
