Form Health in the United States is seeking obesity medicine physicians to provide longitudinal telehealth care for patients with obesity and to collaborate with advanced practice professionals and registered dietitians toward excellent team-based care. Work is remote with flexible schedules.

This Modified FTE (32 hrs/week) role requires AL/MO/MS licensure or LOQ via IMLC and ABOM board certification. You will join a supportive care team, maintain a patient panel, and participate in after-hours

Qualificações

• MD or DO with ABOM diplomate.
• Licensed to practice in AL, MO, or MS.
• LOQ through IMLC or eligible.
• Prior telemedicine experience encouraged.

Responsabilidades

• Provide longitudinal telemedicine obesity care with a dedicated care team.
• Manage complex cases with medical planning and follow-up visits.
• Prescribe non-controlled obesity medications as appropriate.
• Ensure timely clinical documentation and treatment pathways.
• Support APP development and supervise as needed.
• Attend departmental meetings and contribute to quality initiatives.
