Responsibilities

• Provide 1:1 support to a gentleman with learning disabilities and a forensic history.
• Assist with cooking, cleaning and maintaining his home.
• Support to participate in the local community and activities.
• Attend health and medical appointments with him.
• Follow person-centred support plans and risk management strategies.
• Work with the wider support team to implement plans.
