Job Summary: Rehabilitation Specialists provide psychosocial rehabilitative skills training to individuals diagnosed with severe mental illnesses who may also have co-occurring substance use disorders and/or chronic health conditions, utilizing a recovery-based and trauma-informed perspective. They operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams, and cultivate clinical skills by forging therapeutic relationships with diverse community members. Interventions and services are typically provided in the community and require the use of a personal automobile and cell phone.
Rehabilitation Specialists work in partnership with racially and culturally diverse individuals to define and facilitate their desired life outcomes. They participate in diagnostic evaluations and provide assessments of strengths, preferences, functional impairments, and symptoms of psychiatric and/or co-occurring disorders at regular intervals to ensure personalized services. Frequent service coordination with external and internal providers supports individuals in achieving independent functioning. They may deliver therapeutic interventions to foster skills necessary for managing symptoms, obtaining or maintaining employment or housing, and accessing services such as medical care, nutritional assistance, financial aid, transportation, legal aid, and other basic needs. They may also facilitate psychosocial rehabilitative skills training groups and/or utilize other evidence-based interventions or therapeutic modalities. Although not typically LPHA's, staff members work under the supervision of an LPHA who provides ongoing evaluation and oversight of all assessments, diagnoses, and treatments.
On-call responsibilities may be required depending on unit protocols. Additional duties may include providing bilingual communication services, requiring the completion of a language competency test.
Regular, predictable attendance is an essential requirement. Other duties may be assigned, and a flexible work schedule may be necessary.

$23.25 - $24.41 per hour based on education.
