HCA Florida Blake Hospital in Bradenton, FL is seeking an Inpatient Rehabilitation Services Lead to coordinate the IRF-PAI process and provide clinical support for accurate functional status scoring and patient care planning.

The role supports ongoing monitoring of rehab benchmarks, handles confidential patient information, and collaborates with multidisciplinary teams to ensure high-quality rehabilitation services.

Qualifications

• Graduate of an accredited school in a clinical specialty.
• Must be a Florida-licensed professional in a clinical healthcare specialty (RN, Physical Therapist, Occupational Therapist, Speech Language Pathologist).
• 2+ yrs. recent experience as a licensed clinician in an acute-care hospital or inpatient rehabilitation setting.
• Critical thinking, service excellence and good interpersonal communication skills; ability to read/comprehend written instructions and follow verbal instructions; PC skills.

Responsibilities

• Coordinate IRF-PAI completion for timely and accurate results.
• Provide clinical support in data completion to ensure accuracy in the scoring of functional status and reflection of patient care needs.
• Monitor identified benchmarks for the inpatient rehab population.
• Maintain confidentiality of all patient information.
