Job summary

Horizon House, Inc. is seeking a Part-Time Rehabilitation Counselor to provide support, education, counseling, and guidance to children or adolescents with mental health or developmental disabilities.

The role includes collaborating on goals for ISPs and delivering behavioral interventions in home, school, or community settings. Responsibilities include crisis response, maintaining documentation in an Electronic Health Record, and coordinating with a focused team.

Qualifications

• Bachelor’s degree in Human Services/Social Services, Healthcare, or related field with at least 1 year of direct client experience.

Job description

We are seeking qualified candidates for the position of (Part-Time) Rehabilitation Counselor to provide support, education, counseling, and guidance to children or adolescents who have been identified as having mental health or developmental disabilities.

Responsibilities

• Assist with the growth and development of the child/adolescent and the development of therapeutic support skills.
• Provide one-on-one behavioral intervention in the home, school, or community settings in accordance with strategies identified in the Treatment Plan.
• Participate in developing goals with the client for inclusion in the individualized service plan.
• Assist with residential or other services to children with MR/MH as preferred.
• Perform tasks requiring frequent talking and listening, lifting, carrying, folding, kneeling, reaching, stacking, stooping, and bending, including direct intervention with residents.
• Respond to crisis interventions for emergencies, including optional use of passive restraints.
• Maintain daily productivity standards using an Electronic Health Record.

Qualifications

• Bachelor’s Degree in Human Services/Social Services, Healthcare, or related field with at least 1 year of experience working directly with children; or H.S. Diploma/GED plus 12 behavioral health college credit hours and a minimum of 2 years of experience in a human service field involving direct contact with clients.
• Experience in residential or other services to children with MR/MH preferred.
• Good communication and counseling skills.
• Ability to work independently and as part of a focused team.
• Proficiency in electronic health record, Outlook, MS Word, and Excel.
• Fee‑for‑service experience required and ability to maintain daily productivity standards using an Electronic Health Record.
• Valid driver’s license with an acceptable driving record.
