Job summary

Encompass Health Corporation is seeking a Case Manager for a PRN day-shift role (Mon-Thu as needed) to coordinate care from admission to post-discharge. You will work with an interdisciplinary team to guide treatment plans and ensure timely discharge, while educating patients and families about rehabilitation.

The role emphasizes regulatory knowledge, documentation accuracy, and active participation in utilization reviews and discharge planning.

Qualifications

• Must be qualified to independently complete an assessment within the scope of practice.
• Active license if licensure is required for one's discipline.
• CCM or ACM certification required or must be obtained within two years of entry.

Responsibilities

• Coordinate patient care from admission to post-discharge.
• Guide treatment plans with an interdisciplinary team.
• Establish tentative discharge plan and contingency plans.
• Monitor patient experience, quality, timeliness and payor expectations.
• Facilitate team conferences and document plan modifications.
• Complete case management addendums and required documentation.
• Participate in utilization review and ACT meetings.
• Educate patient and family on rehabilitation and the Case Manager role.
