Job summary

Baylor Scott & White Institute for Rehabilitation in Dallas, TX is seeking a Case Manager to join our interdisciplinary team and support clinical and psychosocial aspects of neurologic patient care.

The ideal candidate will have medical or rehabilitation setting experience, performing discharge planning, post-acute care planning, payer/referral management, and patient/family education with access to community resources.

The ideal candidate will have experience in a medical or rehabilitation setting and be able to address not only clinical needs but also the psychosocial, functional and community reintegration factors that impact recovery and long-term outcomes for a complex neurologic patient population.

The primary job functions in for the Case Management role include:

Minimum Qualifications

• Current Licensure per state guidelines in clinical or related discipline OR a Bachelor's or Master's in a health or human services discipline.

Preferred Qualifications

• Previous experience in Case Management and Discharge Planning preferred.
• CCM Certification Preferred.
