Job summary

Centre for Neuro Skills in Bakersfield, CA seeks a Clinical Case Manager to develop, implement, and manage all clinical aspects of case management across clinic and residential sites. The role requires coordinating care and collaborating with rehabilitation teams.

The position is full-time, primarily weekdays, with some on-call duties. A Bachelor’s degree is required; CCM certification is preferred/required upon eligibility; 3–5 years in rehab is expected.

Qualifications

• Passion for Care: deep understanding of the care requirements and effective communication with patients and families.
• Leadership Skills: ability to recruit, train, and evaluate qualified staff and make tough decisions when needed.
• Educational Background: Bachelor's degree required; Master’s or licensure preferred but experience can offset level.
• Work Experience: 3–5 years in rehabilitation or related field.
• Specialized Knowledge: CCM preferred; CCM required upon eligibility; supervisory experience strongly valued.

Responsibilities

• Collect information about the patient’s situation and functioning to identify needs and develop a comprehensive case management plan.
• Plan objectives, goals, and actions to meet the patient’s needs; coordinate with staff.
• Communicate with external stakeholders and families to ensure service coordination.
• Implement case management activities to achieve plan goals.
• Evaluate plan effectiveness at intervals and modify the plan as needed.
