Job summary

Complete Care Centers is seeking a Care Coordinator (Rehab) to support patient recovery through coordinated therapies, accurate documentation in the EMR, and compassionate, patient-centered care.

You will collaborate with providers to keep treatment plans on track, educate patients on exercises and DME, and help deliver a seamless, high-quality clinic experience. No healthcare experience? training is provided to develop your skills and grow with the team.

Responsibilities

• Deliver provider-directed rehabilitation therapies safely and accurately.
• Prepare patients for therapy and explain what to expect.
• Help patients stay on track with their prescribed treatment plans.
• Document therapies accurately in the electronic medical record (EMR).
• Educate patients on therapy exercises and the proper use of Durable Medical Equipment (DME).
• Monitor patient progress and communicate concerns to providers when appropriate.
• Coordinate with providers and teammates to deliver a seamless patient experience.
• Maintain an organized treatment area while supporting efficient daily clinic operations.
• Deliver every patient interaction with professionalism, empathy, and a commitment to excellence.
• Look for opportunities to improve processes and enhance the patient experience.
