Job summary

Brooks Home Health in Gainesville, FL is seeking a registered nurse to implement and manage patient care plans in collaboration with a multidisciplinary team. The role emphasizes immediate admission-to-discharge planning, individualized goals, and independence for patients with disabilities or chronic conditions, applying evidence-based practice and nursing theory.

You will document outcomes, perform medication reconciliation, administer treatments, and educate patients and families.

Qualifications

• Registered Nurse with a State of Florida license.
• Associate, Bachelors, or Master’s degree in Nursing from an accredited school.
• Basic Life Support (BLS) certification required.

Responsibilities

• Develops and documents comprehensive nursing plans of care.
• Documents findings and assesses patient needs within 24 hours of admission.
• Coordinates with multidisciplinary team to plan care and discharge.
• Administers medications and treatments, monitors responses.
• Performs medication reconciliation at SN visits and reports changes.
