Job summary
Trio Health Partners is seeking a Physiatry Nurse Practitioner (Skilled Nursing Facility) to join our rehab-focused team. You will work with a supervising Physiatrist and interdisciplinary staff to manage medical and rehabilitative needs of geriatric and post-acute patients.
Responsibilities include comprehensive rehab assessments, treatment planning, follow-ups, and coordination with therapy teams. Flexible full-time schedules with no nights or weekends and a monthly bonus opportunity are
Qualifications
- Active and unrestricted Nurse Practitioner license in Illinois
- Board-certified as Adult, Geriatric, or Family NP
- Master's Degree in Nursing from an accredited program
- Experience in PM&R, geriatrics, SNF settings preferred
Responsibilities
- Perform comprehensive rehab-focused assessments and mobility goals
- Develop and update personalized treatment plans with supervising physician
- Conduct regular follow-ups to monitor progress and modify care plans
- Collaborate with therapy teams and participate in rounds
- Document visits and outcomes clearly in the EMR
- Support discharge planning and care transitions to ensure continuity
