Job summary

Trio Health Partners is seeking a Physiatry Nurse Practitioner to join our team at a Skilled Nursing Facility. This in-person, part-time role partners with a supervising Physiatrist to manage rehab-focused care for geriatric and post-acute patients.

You will perform comprehensive rehab assessments, develop treatment plans, conduct follow-ups, and educate patients and families while coordinating with therapy teams and the EMR to ensure smooth transitions and optimal outcomes.

Qualifications

• Active IL NP license and board certification in PM&R, geriatrics, or general internal medicine preferred.
• Master’s Degree in Nursing from an accredited program.
• Experience in PM&R, geriatrics, SNF settings preferred.

Responsibilities

• Perform comprehensive rehab-focused assessments and mobility goals.
• Develop and update individualized treatment plans with supervising Physiatrist.
• Conduct regular follow-ups to monitor progress and modify plans of care.
• Participate in interdisciplinary care team meetings and patient rounds.
• Collaborate with therapy teams to track rehabilitation milestones.
• Educate patients and families about diagnoses, goals, and functional strategies.
• Document visits and outcomes clearly in the EMR.
• Support discharge planning and care transitions to maintain progress.
• Serve as a clinical resource to staff for musculoskeletal/neurologic conditions.
