Job summary

TriStar Horizon Medical Center in Dickson, Tennessee, seeks a licensed Physical Therapist to provide high-quality, patient-centered care across the continuum of therapy. You will perform assessments, develop and implement individualized plans, and deliver or direct therapeutic services in coordination with the physician and care team.

Key duties include evaluating status, planning care, instructing patients and families, prioritizing caseload, and coordinating handoffs.

Qualifications

• Licensed Physical Therapist in good standing.
• Master's degree in Physical Therapy or related field.
• Basic Cardiac Life Support (BCLS) certification obtained within 30 days of start date.

Responsibilities

• Provide evaluations and treatment interventions within scope of practice and professional ethics.
• Assess and evaluate therapeutic/rehabilitative/functional status and contribute to the total care plan.
• Deliver or direct high-quality physical therapy to support patients throughout the continuum of care.
• Instruct patients, families, and caregivers in therapies, exercises, devices to be used outside of sessions.
• Prioritize caseload, determine daily staffing needs, and coordinate assignments.

Job Summary And Qualifications

The Physical Therapist (PT) provides high-quality, patient-centered care by managing the complete process of therapeutic care of an assigned group of patients in coordination with the physician and the patient care team. The Physical Therapist completes patient assessments and participates in developing and implementing individualized care plans focusing on goal achievement, patient engagement, and positive patient experience. The Physical Therapist provides or directs a majority of the patient’s physical therapy to support patients throughout the continuum of care.

What you will do in this role

• You will provide evaluations and treatment interventions within the scope of practice established by law and professional code of ethics.
• You will assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care.
• You will provide purposeful and skilled therapeutic care at patient’s home, using procedures and techniques to produce a change in condition/function consistent with the physician’s plan of care.
• You will instruct patient/family/caregiver in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.
• You will prioritize caseload, determine daily staffing needs and coordinate assignments/handoffs.

• Masters Degree
• Basic Cardiac Life Support, or BCLS Instructor must be obtained within 30 days of employment start date
