Job summary

Cvhcare in California is seeking a Patient Care Coordinator to extend impact beyond direct patient care by coordinating multidisciplinary home health services.

In this role you will liaise with patients, families, providers, and caregivers; manage referrals, authorizations, scheduling, and care plan coordination; participate in interdisciplinary case conferences; monitor progress, and support regulatory compliance to deliver high-quality outcomes.

Qualifications

• Active California PT, OT, or SLP license required.
• Current BLS certification.
• Home Health experience preferred.
• Knowledge of Medicare regulations and OASIS preferred.
• Strong organizational, communication, and computer skills.
• Leadership or care coordination experience is a plus.
