All Care and Comcare Home Health in California is seeking a compassionate Field Physical Therapist to join our growing home-health team. You will deliver high-quality, individualized PT services in patients’ homes while enjoying a flexible schedule and supportive environment.

Responsibilities include SOC, ROC, Recert, and DC evaluations; plan interventions with physician orders; educate patients and families; travel to patient homes; collaborate with the interdisciplinary team to meet PT goals.

Qualifications

• Active California Physical Therapy license.
• Graduate of an accredited Physical Therapy program.
• Current CPR certification.
• Valid California driver’s license and auto insurance.
• Two professional references.
• Strong documentation and organizational skills.
