Job summary

PeaceHealth Springfield Rehabilitation Hospital, part of Lifepoint Health, is opening a brand-new inpatient rehabilitation facility in 2026. We are seeking an Outcomes Manager to join our team to support patient-centered outcomes and data integrity.

You will coordinate PPD data collection with the interdisciplinary team, ensure complete QI documentation, and complete IRF-PAIs within CMS timeframes using CMS-approved software. Maintain logs and ensure accuracy across the patient roster.

Qualifications

• QI competency required.
• Strong clinical assessment/analysis, and verbal and written communication.
• Working knowledge of state and federal guidelines; familiarity with healthcare workflows and documentation.
• Basic computer and data entry proficiency required.
• Ability to collaborate across disciplines.

Responsibilities

• Coordinate PPD data collection with the interdisciplinary team.
• Ensure supportive documentation for QIs is complete in the medical record.
• Complete admission and discharge IRF-PAIs within CMS-designated timeframes.
• Abstract required information from medical records and summarize on the IRF-PAI.
• Enter data into the CMS-accepted software/program within designated timeframes.
• Maintain and update the IRF-PAI log; verify accuracy against the patient location roster.
