Job summary

Lewis-Gale Medical Center in Salem, VA is hiring a Clinical Rehabilitation Specialist to coordinate between case management, rehab operations, physicians, payors and referral sources to ensure appropriate patient placement.

You will perform screenings, support admission decisions, and educate teams and families while developing referral networks for inpatient acute rehab. This role emphasizes collaboration and patient-centered care.

Qualifications

• Licensed Professional in a clinical health care specialty.
• Current state licensure in clinical practice area or eligible for state license transfer.
• Graduate of an accredited school in clinical specialty.
• Minimum 2 years recent clinical care experience.

Responsibilities

• Coordinate intake and pre-admission screening for rehab program.
• Review patients daily with Case Management to determine admission.
• Provide information about inpatient rehab to Case Management, patients and families.
• Establish relationships with referral sources and physicians to support rehab referrals.
• Educate referral sources on inpatient rehab criteria and processes.
• Maintain timely follow-up communications with referral sources.
• Assist in developing annual strategic plan for the Rehab Unit and outreach tools.
