Job summary

The Legacy at Boca Raton Rehabilitation and Nursing Center in Boca Raton, FL is seeking a dedicated Speech Language Pathologist (SLP) to evaluate and treat residents with speech, language, voice and swallowing disorders. You will work within a collaborative rehabilitation team to promote residents’ independence and quality of life.

Responsibilities include developing individualized treatment plans, delivering therapy services, monitoring progress and documenting outcomes.

Qualifications

• Active Florida SLP license in good standing.
• CCC preferred or board cert in speech-language pathology.

Responsibilities

• Evaluate residents with speech, language, cognitive, communication and swallowing disorders.
• Develop and implement individualized treatment plans based on resident needs.
• Provide speech therapy services to improve communication, cognition and swallowing.
• Monitor progress and adjust treatment plans as needed.
• Collaborate with physicians, nursing and interdisciplinary team to support outcomes.
• Maintain accurate clinical documentation per regulatory standards.
• Educate residents and families about therapy goals and progress.
• Participate in care planning and discharge planning processes.
