Job summary

The Legacy at Boca Raton Rehabilitation and Nursing Center in Boca Raton, FL is seeking a dedicated Speech Language Pathologist (SLP) to evaluate and treat residents with speech, language, voice and swallowing disorders.

You will join a collaborative rehabilitation team focused on resident-centered care and positive outcomes, providing comprehensive therapy and contributing to discharge planning and family education.

Qualifications

• Active Florida Speech Language Pathologist license in good standing.
• Certificate of Clinical Competence (CCC) preferred.
• Previous experience in a skilled nursing facility, rehabilitation center, or healthcare setting preferred.
• Strong clinical assessment and treatment planning skills.
• Excellent communication and interpersonal abilities.
• Ability to work effectively within an interdisciplinary team.
• Compassionate, dependable, and resident-focused.

Responsibilities

• Evaluate residents with speech, language, cognitive, communication, and swallowing disorders
• Develop and implement individualized treatment plans based on resident needs and goals
• Provide speech therapy services to improve communication, cognition, and swallowing function
• Monitor resident progress and adjust treatment plans as appropriate
• Collaborate with physicians, nursing staff, and interdisciplinary team members to support positive resident outcomes
• Maintain accurate and timely clinical documentation in accordance with regulatory and facility standards
• Educate residents and families regarding therapy programs and treatment goals
• Participate in care planning meetings and discharge planning processes
• Ensure compliance with all federal, state, and facility policies and procedures
• Maintain a safe, organized, and supportive therapy environment
