Job summary

The Legacy at Boca Raton Rehabilitation and Nursing Center in Boca Raton, FL is seeking a dedicated Speech Language Pathologist (SLP) to join our rehabilitation team. The SLP will evaluate and treat residents with speech, language, cognitive, and swallowing disorders, helping them achieve their highest level of independence and quality of life.

Responsibilities include developing individualized treatment plans, delivering evidence-based therapy, and collaborating with physicians and nursing

Qualifications

• Active Florida Speech Language Pathologist license in good standing.
• Certificate of Clinical Competence (CCC) preferred.
• Experience in skilled nursing facility or rehabilitation setting preferred.
• Strong clinical assessment and treatment planning skills.
• Excellent communication and interpersonal abilities.

Responsibilities

• Evaluate residents with speech, language, cognitive, communication, and swallowing disorders
• Develop and implement individualized treatment plans based on resident needs and goals
• Provide speech therapy services to improve communication, cognition, and swallowing function
• Monitor resident progress and adjust treatment plans as appropriate
• Collaborate with physicians, nursing staff, and interdisciplinary team to support positive resident outcomes
• Maintain accurate and timely clinical documentation in accordance with regulatory and facility standards
• Educate residents and families regarding therapy programs and goals
• Participate in care planning and discharge planning processes
• Ensure compliance with all policies and procedures
• Maintain a safe, organized therapy environment
