Job summary

Legacy Healthcare seeks a licensed PT/OT/SLP to serve as the primary Legacy representative at a Cary, NC facility. You will promote Legacy policies, assess residents, and coordinate rehab services to support optimal patient outcomes.

You will lead clinical program development, supervise staff, manage caseloads, and ensure compliance with state and federal standards while collaborating with facility personnel.

Qualifications

  • Licensed and registered as PT/OT/SLP in the state of assignment.
  • Experience in long-term care or senior living preferred.

Responsibilities

  • Serve as the primary Legacy representative at the facility.
  • Promote Legacy policies and clinical programs.
  • Build rapport and positive working relationships with facility personnel and Legacy employees.
  • Discover the needs of facility personnel and residents.
  • Oversee internal marketing efforts and keep community leaders informed about Legacy programs and offerings.
  • Manage site productivity and make necessary changes to achieve budget.
  • Maintain a caseload to help the rehab department meet standards.
  • Assess patient needs and develop clinical programs appropriate for facility census.
  • Direct and train others in clinical service delivery across disciplines.
  • Coordinate the rehab department daily with facility staff to plan and implement therapy programming.
  • Participate in clinical outcomes reporting and share results with the customer.
  • Manage staff schedules to meet caseload requirements and use scheduling tools for patient schedules.
  • Manage CliniFax and physician signatures on therapy plans of care.
  • Maintain team relationships and open communication among rehab staff, facility staff, and Legacy corporate personnel.
  • Assist with staff orientation and training to meet company, state, and federal standards.

Job description

Requirements
  • Clinical background as a Physical Therapist (PT), Physical Therapy Assistant (PTA), Occupational Therapist (OT), Certified Occupational Therapy Assistant (COTA), or Speech Language Pathologist (SLP)
  • Three to five years' experience in long-term care (LTC) or senior living
  • Some prior supervisory experience preferred but not required
  • Minimum of a degree from an accredited college or university or equivalent experience, such as a degree in PT, PTA, OT, COTA, or SLP
  • Licensed and registered as a PT, PTA, OT, COTA, or SLP in the state where assigned
Core Competencies

Demonstrates expertise in clinical program development and patient assessment, with a strong focus on collaboration and communication among multidisciplinary teams. Holds a valid license as a Physical Therapist, Occupational Therapist, or Speech Language Pathologist, ensuring compliance with state and federal standards.
