Job summary

Legacy seeks a dedicated rehab leader to serve as the primary representative in a long-term care facility in Knoxville, TN. You will promote Legacy programs, coordinate therapy services, and manage staff while ensuring compliant, patient-centered care.

The role requires a PT/OT/SLP license, 3–5 years LTC experience, and strong leadership skills to train teams and drive outcomes in a clinical setting.

Qualifications

• Three to five years’ experience in long-term care (LTC) or senior living.
• Minimum of a degree from accredited college or university in PT/OT/COTA/SLP or equivalent experience.
• Licensed and registered in the state where assigned (PT/OT/COTA/SLP).

Responsibilities

• Serve as the primary Legacy representative at the facility.
• Promote Legacy policies, clinical programs, and the mission to help aging adults live safe, healthy lives.
• Create rapport and positive working relationships with facility personnel and Legacy employees.
• Oversee internal marketing efforts and communicate Legacy programs and offerings to community leaders.
• Manage site productivity and make necessary changes to achieve budget.
• Maintain a caseload to assist the rehab department in meeting standards.
• Assess patient needs and develop clinical programs appropriate for facility census.
• Direct and train others in clinical service delivery across disciplines.
• Coordinate the rehab department daily with facility staff to plan and implement therapy programming.
• Participate in clinical outcomes reporting and share results with the customer.
• Manage staff schedules and use scheduling tools for patient schedules.
• Manage CliniFax and physician signatures on therapy plans of care.
• Maintain team relationships and open communication among rehab staff, facility staff, and Legacy corporate personnel.
• Assist with staff orientation and training to meet company, state, and federal standards.

Requirements

• Clinical background as a Physical Therapist (PT), Physical Therapy Assistant (PTA), Occupational Therapist (OT), Certified Occupational Therapy Assistant (COTA), or Speech Language Pathologies (SLP)
• Three to five years’ experience in long-term care (LTC) or senior living
• Some prior supervisory experience preferred but not required
• Minimum of a degree from accredited college or university or equivalent experience (i.e., degree in PT, PTA, OT, COTA, SLP)
• Licensed and registered (PT, PTA, OT, COTA, SLP) in state where assigned
