Job summary

Key Rehab is seeking a full-time Program Coordinator, OT in a SNF setting. You will support clinicians, coordinate staffing and scheduling, and help reduce administrative burden under regional supervision.

Required: completion of an accredited PTA or OTA program and a valid PTA/OTA license; CPR, driver license, and insurance as required by state law. Prior long-term care experience preferred.

Qualifications

• Preferred LTC experience and service coordination.
• Current PTA or OTA license to practice in the state.
• CPR as required by community.
• Driver license and car insurance as required by state law.
• Other items as required by standards.

Responsibilities

• Liaise between Key Rehab and community staff while providing therapy.

Overview

Key Rehab is looking for a Full time, Program Coordinator, OT to join our team in a SNF setting.

Education

• Required: Completion of an accredited PTA or OTA program

Experience

• Preferred: Prior experience in long term care setting and service coordination

Licenses/Certificates

• Required: Current PTA or OTA license to practice in the state(s) assigned
• Required: CPR, as required by community
• Required: Valid driver license and car insurance as required by state law.
• Required: Other items as required by federal, state and local standards

Responsibilities

In addition to providing therapy under their disciplinary scope of practice (OTA or PTA), the Program Coordinator serves as the liaison between Key Rehab and community staff. This position is instrumental in minimizing administrative burden and providing support to allow clinicians' to deliver therapy services. This position also coordinates staffing and daily scheduling to meet key performance indicators and expectations under the direction of a Regional Supervisor.

• Exceptional Communication: Outstanding oral and written communication skills for clear and effective interaction with patients and colleagues.
• Experience with Geriatric Population: Prior experience working with elderly patients, demonstrating an understanding of their unique needs.
• Knowledge of Medicare Billing Regulations: Familiarity with current Medicare billing practices and guidelines.
