Job Description

Coordinate care services, conduct utilization reviews, and provide case management for patients at Spain Rehabilitation Center. Support patients and families through the rehabilitation process, assess patient status, and facilitate discharge planning to ensure smooth transitions across care continuum.

Qualifications

• Bachelor’s degree in Nursing, Occupational Therapy, Physical Therapy, or Speech-Language Pathology from an accredited program.
• Three (3) years of related experience in a rehabilitation environment and/or case management preferred.
• Current license or eligibility to be licensed as a Speech-Language Pathologist, Occupational Therapist, Physical Therapist, or Registered Nurse (RN) in Alabama.
• CPR certification required.
