Job summary

Responsibilities

• Coordinate health care decisions to assure treatment plans that improve quality and outcomes.
• Coordinate care across the continuum and promote cost-effective care within the time frame.
• Assure payments of hospital-based services meeting patient-related utilization management criteria.
• Implement safe discharge plans; assess psychosocial needs as part of discharge planning.
