Qualifications

• Case management and discharge planning experience preferred.
• CCM certification preferred.

Responsibilities

• Coordinate care across the continuum and promote cost-effective care.
• Assure hospital-based services payments meet utilization criteria.
• Assess psychosocial needs and provide discharge planning interventions.
