The role emphasizes adherence to regulatory standards, payor requirements, and ongoing collaboration with clinical teams to optimize patient outcomes and satisfaction in a leading inpatient rehabilitation system.

Qualifications

• Qualified to independently complete assessments within scope of practice.
• Active license required per state regulations.
• CCM or ACM certification must be obtained within two years of entry.

Responsibilities

• Coordinate discharge planning and contingency plans with the interdisciplinary team.
• Plan and execute patient discharge experiences and treatment plan modifications.
• Complete case management documentation and addendums on time.
• Monitor regulatory compliance, payor requirements, and quality indicators.
• Participate in utilization review, ACT meetings, and on-call schedule as needed.
