Altru Rehabilitation Hospital seeks a compassionate Case Manager to coordinate care for inpatient and outpatient rehab patients in our Healthcare & Medical Services department.

You will manage caseloads from admission through discharge, complete assessments, develop individualized care plans, facilitate interdisciplinary team collaboration, and secure post-acute services and community resources to support long-term recovery.

Qualifications

• Coordinate rehab care from admission to discharge.
• Develop individualized care plans and adjust as needed.
• Collaborate with physicians, therapists, and nursing staff.
• Educate patients and families on resources and recovery goals.

Responsabilités

• Coordinate rehab caseload from admission through discharge.
• Develop and update individualized care plans.
• Facilitate interdisciplinary team meetings and services.
• Conduct psychosocial assessments and discharge planning.
• Educate patients and families on resources and recovery goals.
• Collaborate with physicians, therapists, and nursing staff.
• Document assessments, plans, and outcomes in the EMR.
