Job summary

Shreveport Rehabilitation Hospital is seeking a Case Manager to collaborate with physicians and manage individualized patient programs across the continuum of care. You will coordinate clinical and financial aspects of treatment plans to ensure timely, cost-effective services for rehabilitation patients with various conditions, while adhering to CMS length of stay guidelines.

The role requires RN or LSW licensure with hospital experience preferred and case management certification as a plus.

Qualifications

• Current state licensure as a Registered Nurse or Licensed Social Worker.
• 2 years hospital experience preferred.
• Certification in case management preferred.

Responsibilities

• Provide individual program management for each patient to ensure progression through the continuum of care.
• Monitor and coordinate clinical and financial aspects of treatment plans for assigned patients.
• Coordinate length of stay management within CMS guidelines and the 60% compliance threshold.
