Job summary

Texas Rehabilitation of Plano, opening a brand-new hospital, seeks a Case Manager to support the inpatient rehab team. You will coordinate care, document IPoCs, and lead family conferences while collaborating with nursing and therapy to optimize discharge planning.

Qualified candidates are RNs or licensed social workers with at least 2 years in inpatient case management; related certifications are highly valued. This role offers comprehensive benefits and career growth opportunities.

Qualifications

• Current Registered Nurse or Social Work licensure is required or preferred.
• Minimum 2 years social work or case management in an inpatient setting preferred.
• Certifications in Case Management or Rehabilitation Nursing (CCM/ARN/ACM or RN-BC) are preferred.

Responsibilities

• Completes departmental orientation, initial and annual competencies.
• Assists with department-specific performance improvement initiatives collecting and reporting data as requested by supervisor.
• Consults other departmental staff to collaborate in patient care delivery, identify barriers to care and discharge plans, and develop solutions.
• Completes documentation per workflow timeline and content requirements including IPoC per CMS guidelines.
• Schedules family conferences and communicates with caregiver following each team conference and more often as needed to keep patient and designated caregiver informed of progress and plans for discharge.
• Coordinates weekly patient care team conferences to facilitate development, monitoring and refinement of treatment plan to achieve identified patient goals and outcomes.
• Reviews the patient's assigned CMG and helps the team identify any potential missed comorbid conditions that are actively being treated during the patient's stay.
• Communicates any findings to the HIM team.
• Communicates effectively with nursing, therapy and other ancillary departments to ensure proper utilization.
