Qualifications

• Current Registered Nurse or Social Work licensure or Healthcare professional licensure as above.
• Minimum of 2 years social work or case management experience in an inpatient setting preferred.

Responsibilities

• Completes departmental orientation, initial and annual competencies.
• Assists with departmental performance improvement initiatives, collecting and reporting data.
• Consults with other staff to collaborate in patient care, identify barriers to care and discharge plans.
• Completes documentation per workflow including IPoC per CMS guidelines.
• Schedules family conferences and communicates progress and discharge plans.
• Coordinates weekly patient care team conferences to refine treatment plans.
• Communicates findings to the HIM team and engages with nursing/therapy teams.

Education

RN license or Social Work licensure or Healthcare professional licensure (e.g., Respiratory Therapist, Physical Therapist, Speech Language Pathologist, Occupational Therapist)
