Job summary

Methodist Rehabilitation Hospital, operated with Lifepoint Health and Methodist Health System, is seeking a dedicated Case Manager to join our team in Dallas. You will collaborate with an interdisciplinary team, document IPoC per CMS guidelines, and support discharge planning to help patients transition safely.

The role requires RN, SW, RT, PT, OT, or SLP licensure and at least 2 years in social work or case management in inpatient settings; strong communication and MS Office skills are

Qualifications

• Licensure as RN, SW, RT, PT, OT or SLP required.
• Minimum 2 years in social work or case management in inpatient setting preferred.
• English communication proficiency required; CMS guidelines knowledge a plus.

Responsibilities

• Complete orientation and annual competencies.
• Collaborate with interdisciplinary team to identify barriers to care and discharge solutions.
• Document IPoC per CMS guidelines within workflow timelines.
• Schedule family conferences and communicate progress with caregivers.
• Coordinate weekly interdisciplinary team conferences to monitor goals.
